AFP National Police Check Information Intake

Share all required personal, contact, and identification details so our team can submit an Australian Federal Police National Police Check on your behalf. Applicant Identity

Provide your primary identity details matching travel documents. Ensure the spelling aligns with your passport.

If you have held multiple nationalities or passports in the last 10 years, please list them in the supporting notes at the end.

Name History & Supporting Details

AFP requires every name you have been known by, including maiden names, previous married names, or aliases.[1]

Add every previous name exactly as it appeared on official documents.

Residential History (Last 10 Years)

Provide consecutive addresses covering at least the past 10 years. Include overseas addresses as well.[1]

Tips for accuracy:

  • Ensure there are no date gaps between addresses.
  • Include the month and year even if you do not recall the exact day.
  • If you resided overseas, include those addresses with country indicated.

Identification Documents & Declarations

AFP requires at least 100 points of identification evidence.[1] Upload clear scans or photos of each document.
